Absence Request Form
Must be completed 10 days prior to an educational trip.
Activity Request Form
Must be completed when planning an event or activity on LVA property.
Athletics Confirmation Sheet
Must be completed prior to participating in sports at the home district.
Free/Reduced Lunch Application
Must be completed and returned to Jamie Link, Lunch Program Coordinator
LVA Clothing Order Form
Also used for K-5 PE uniforms. If you are in need of a PE uniform for a student in grades 6-12, please contact his/her PE teacher.
Subscription Form
Use this form to let us know how you would like to receive information from LVA (Newsflash, announcements, etc.).
Transportation Form
Must be completed each year to request transportation from the sending district.

2010-2011 Lunch Program
Lunch order forms are due with payment by the 15th of the previous month. Menus are sent home monthly with students or can be printed from the links below.
